個人健康管理整合服務

將個人健康數據集中並整合，提供紙本1 份+ PDF電子檔，以便利有需要人士使用(移居國外、保險投保或索償等)。

檢視目前面對的健康問題，了解潛在的根本原因。

檢視健康檢查數字(包括體脂、血糖指數和紀錄、血液檢查、藥物使用情況等)，對未來進展作評估並提供建議。

醫療保險索償表格及資料整合

(包括住院醫保、意外醫保、癌症醫保、公司醫保程序)

服務包括：

在投保人完成治療後，輔助整理集齊醫護人員填寫文件、索償申請表格、主診醫生轉介信、醫療收據(包括由醫生、跌打醫師、針灸醫師等發出的醫療開支收據，清晰列明病人姓名、病症名稱、診斷日期、結果、每項醫療收費金額、並附上醫生簽署及印鑑)、手術報告、化驗報告、電腦掃描、影像報告及所需文件正本。

如在醫院管理局轄下政府醫院，則需要完整出院撮要。

**服務範圍只包括索償表格輔導及所需文件資料，並不包括申請遞交，亦不影響最終賠償結果。

**所有索償申請由投保人/申請人自行遞交及索償跟進。

Personal Healthcare Integration Service

Centralize and integrate personal health data, providing one paper copy + a PDF electronic file for the convenience of those in need (e.g., for emigration, insurance applications, or claims).

Review current health issues faced and understand potential underlying causes. 

Examine health check metrics (including body fat, blood sugar levels and records, blood test results, medication usage, etc.), assess future progress, and provide recommendations.

Integration of Medical Insurance Claim Forms and Data

(Including inpatient medical insurance, accident insurance, cancer insurance, and corporate medical insurance procedures)

Services include:

After the policyholder completes treatment, assist in organizing and collecting documents filled out by healthcare professionals, claim application forms, referral letters from attending doctors, medical receipts (including receipts for medical expenses issued by doctors, traditional Chinese medicine practitioners, acupuncturists, etc., clearly stating the patient’s name, condition, diagnosis date, results, individual medical fees, and accompanied by the doctor’s signature and seal), surgical reports, laboratory reports, CT scans, imaging reports, and original copies of required documents.

For cases under the Hospital Authority’s public hospitals, a complete discharge summary is required.

** Our service offers guidance on claim forms and required documentation. It does not include submission of applications nor affect the final compensation outcome. 

** All claim applications must be submitted and followed up by the policyholder/applicant themselves.
